In the absence of bed side record, the patient medication administration records (MAR) does not provide adequate inflation about the patient medication and mediation protocol. Most education errors occur due to omission and lack of communication. Nurses can reduce medication errors by informing and documenting omissions in the medical records can help prevent accidental overdose of medication. according to da Silva, &, Krishnamurthy (2016), emergency departments is the third most common source of medication errors and over 3.5 million physician office visits as a result of adverse drug gravitons. Among the special population, the elderly in the emergency departments are associated with more medication errors because they lack medical knowledge and cannot access their bedside records. On in every 30 hospitals admissions is associated with adverse drug events 42% of temporary harm events wee relate to medication 50% of these event were considered preventable if proper documentation was and bed side reporting was given primacy. Additionally, the economic impact of these medication errors was estimated at $3.5 billion per year while the adverse drug event contributed to atheist 2milion hospital stays prolonging the length of stay n hospitals by 1 to 6 days

Bedside reports is an improvement over the traditional report at nurses station as it is convenient to both the new shift nurse and the patient and details how to implement the bedside report. Through the bedside reports, patients are better informed. Secondly, beside reports are also assisted with patient satisfaction as compared to the traditional nurses report left at the nurses stations.

Implementing standardized and structured shift handover protocols can significantly improve nurses practices safe and improve patient safety.
